Referrals:
If your insurance requires a referral to see a specialist, please request it from your primary care provider and allow sufficient time for processing before your appointment. Most providers require 48 hours notice for referrals.

Minors:
Please remember that patients under the age of 18 yrs must be accompanied by their parent or guardian at their first visit, subsequent unaccompanied visits will require a signed consent.
